Ahlam Hoops Sports Waiver Form
Waiver Section – Please Read, Initial and Sign**READ CAREFULLY BEFORE SIGNING**
Release and Indemnity (Adult – 18 years of age or over; Minor – under 18 years of age)
In consideration of my, or my child’s, participation in the above-referenced Activity/Sport Type, I agree to assume the risks incidental to such participation and use which risks may include, among other things, major and/or minor physical injury, including death or disfigurement. The risks may include, but are not limited to, those caused by aggressive play, unsafe play, terrain, facilities, temperature, weather, condition of participants, equipment, vehicular traffic, and actions of other people including, but not limited to, participants, volunteers, spectators, coaches, and lack of hydration, or general lack of physical conditioning. I acknowledge that I have or will inspect the facilities and equipment to be utilized in conjunction with the Activity and, if I believe any unsafe condition exists, I will immediately advise an activity official of such condition and will refuse participation until such condition is corrected.
On my own or my child’s behalf, and on behalf of my or my child’s heirs, executors, administrators, and next of kin:
I hereby waive, release, covenant not to sue, and forever discharge Ahlam Soccer Academy, together with any and all related entities, and the officers, directors, employees, and volunteers of any of them, and any and all Activity sponsors or charities, and each of their parent, related and affiliated companies, and the officers, directors, employees, agents, Board of Supervisors, representatives, successors, and assigns of each of the foregoing entities (collectively, the “Released Parties”), of and from all liabilities, claims, actions, damages, costs, or expenses of any nature arising out of or in any way connected with my or my child’s participation in such activities.
I further agree to indemnify and hold each of the Released Parties harmless against any and all such liabilities, claims, actions, damages, costs, or expenses including, but not limited to, all attorney's fees and disbursements up through and including any appeal. I understand that this Release and Indemnity Agreement includes any claims based on the negligence, carelessness, action, or inaction, of any of the above Released Parties, and from dangerous or defective equipment or property owned, maintained or controlled by Released Parties and covers bodily injury (including death), property damage, and loss by theft or otherwise, whether suffered by me or my child either before or after such participation.
I declare that I, or my child, are physically fit with the skill level required to participate in this Activity, and have not been advised to not participate by a qualified medical professional.
I certify that there are no health-related reasons or problems which preclude my, or my child's participation. I further authorize medical treatment for myself, or my child, at my cost, if the need arises.
I also understand that my child or I may be required to leave the Activity venue should my child or I exhibit undesirable conduct.
This Waiver and Permission Form shall be governed by the laws of the State of California, and any legal action relating to or arising out of this Waiver and Permission Form shall be commenced exclusively in the Orange County Superior Court, California (or if such Court shall not have jurisdiction over the subject matter thereof, then to such other court sitting in said county and having subject matter jurisdiction), and I specifically waive the right to trial by jury. I certify I am 18 years of age or older and if I am executing this Waiver and Permission Form on behalf of my child, the information set forth above pertaining to my child is true and accurate and within my legal authority.
Social Media Wavier
By enrolling in Ahlam programs, you consent to the use of photographs, videos, and other media taken during academy events and training sessions featuring participants. This waiver allows us to use these images and recordings for promotional purposes, including but not limited to social media, the academy’s website, newsletters, and other marketing materials.
Terms: Media Use may use images and videos of participants to showcase academy activities, events, and accomplishments. Privacy Protection, Personal details such as full names, contact information, or specific identifying details will not be shared without further consent.
Opt-Out If you prefer that images or videos of your child not be used for promotional purposes, please inform us in writing at ahlamsocceracademy@gmail.com prior to the season start. We will make every effort to respect your preference. By agreeing to this waiver, you acknowledge and understand that no compensation will be provided for the use of these media, and you release Ahlam from any liability associated with the use of these images.
Ahlam Hoops Practices & Tournaments Code of Conduct
At Ahlam Hoops, we are committed to fostering a safe, respectful, and inclusive environment for all participants, parents, coaches, and spectators. By attending or participating in our practices and tournaments, you agree to the following guidelines:
1. Respect for Participants
All players, regardless of age or skill level, are to be treated with kindness and respect.
Verbal or physical aggression toward players, coaches, referees, or spectators is strictly prohibited.
2. Parent & Spectator Behavior
Parents and spectators are expected to support all players in a positive and encouraging manner.
Under no circumstances should a parent or spectator discipline, scold, or confront a child who is not their own.
Any concerns about player conduct must be brought to the attention of referees, coaches, or Ahlam Soccer Academy staff—not addressed directly.
3. Zero Tolerance for Violence
Any form of physical aggression, intimidation, or threats between adults or children will result in immediate removal from the facility and potential suspension from future events.
Verbal altercations or shouting matches are considered misconduct and will also lead to disciplinary action.
4. Conflict Resolution
Disputes must be handled through appropriate channels: referees, coaches, or Ahlam Soccer Academy staff. Escalating conflicts yourself is not acceptable.
5. Role of Coaches & Referees
Referees and coaches have full authority over the game. Their decisions must be respected without argument or confrontation.
If there are grievances regarding calls or decisions, they should be submitted in writing after the game.
6. Supervision of Children
Parents are responsible for the behavior of their children at all times while on the premises.
Any unsportsmanlike conduct by a child will be addressed by referees or coaches, not other parents or spectators.
7. Consequences for Misconduct
Verbal Warning: For minor infractions such as inappropriate language or unsportsmanlike behavior.
Temporary Suspension: For aggressive behavior, repeated verbal altercations, or failure to comply with staff instructions.
Permanent Ban: For physical violence, harassment, or ongoing disruptive behavior.
8. Reporting Incidents
All incidents must be reported to Ahlam Hoops staff immediately. An incident report will be completed and reviewed to determine the appropriate action.
9. Agreement to Abide by the Code
All players, parents, and spectators must read and sign this Code of Conduct prior to participation in any event hosted by Ahlam Hoops.
By adhering to these guidelines, we ensure a positive, safe, and enjoyable experience for everyone involved. Thank you for your cooperation!
AHLAM HOOPS Refund Policy
At Ahlam, we understand that circumstances may arise requiring members to withdraw from a season. Our refund policy ensures fairness to all families while covering the costs involved in setting up the season’s programs.
Refund Policy Details:
Full Refund
If cancellation is requested at least 15 days prior to the season start date, a full refund of the registration fee will be provided.
Partial Refund
If cancellation is requested within 15 days before the season start, a 50% refund will be issued to cover initial season setup costs.
No Refund
No refunds will be provided if a cancellation request is made after the season start date. Exceptions may be considered for medical reasons with proper documentation, at the academy’s discretion.
Injury or Medical Condition
If a player sustains an injury or has a medical condition that prevents participation, a prorated refund or credit may be issued based on the remaining season, subject to medical documentation.
Weather and Unforeseen Circumstances
Refunds will not be provided for sessions canceled due to weather, facility closures, or other circumstances beyond our control. Makeup sessions or credits may be offered if possible.
